How do I qualify for a Cooper Tire rebate?

Purchase four (4) qualifying new Cooper tires in one transaction between August 25, 2011 and November 7, 2011. Be sure to include your completed rebate request form, along with your sales receipt showing the purchase date, qualifying tire description, price and tire size. Mail to:

US consumers will receive a prepaid VISA® rebate card issued by JPMChase. Canadian consumers will receive a rebate check in Canadian dollars. To be eligible, all rebate requests must be postmarked by December 8, 2011 and received at the fulfillment center no later than December 23, 2011. LATE SUBMISSIONS WILL NOT BE ACCEPTED.

What are the qualifying Cooper tires for this rebate?

The qualifying Cooper tires for this event are: $75 for the Cooper Zeon RS3-A, Cooper Zeon RS3-S, Discoverer H/T, Discoverer STT, Discoverer A/T3, Discoverer CTS, Discoverer LSX**, Discoverer LSX Plus**, Discoverer HTP**, Discoverer ATP** tires. $50 for the CS4 Touring tires. $40 for the Lifeliner GLS and Response Touring** tires. SUBSTITUTIONS ARE NOT ELIGIBLE.

How long will it take to receive my rebate?
Approved rebate requests will be paid within 6-8 weeks of receipt at the fulfillment center. Check the status of your rebate online.
I looked up my status online and it says "your rebate request could not be processed." What does this mean?

This message means the rebate request could not be processed because there are irregularities with your request form or sales receipt. The original paperwork will be mailed to you with a letter of explanation from the fulfillment center. If the request was denied for a reason that can be corrected, you have the opportunity to resubmit corrected documents and still receive a rebate. Please take note of program deadlines when resubmitting.

How do I use the Cooper Tire VISA® prepaid rebate card?
The prepaid rebate card is activated by calling the toll free number provided on the card. It can then be used at any location that accepts VISA® cards. Make note of the authorization code set up during the activation process. You will need the code later to access information about your card. Certain fees apply, so read carefully all information on the card mailer. The card will expire when all funds have been used or 12 months from the month of issue.
How do I check the balance of my VISA® prepaid rebate card?
To check the balance of your VISA® prepaid rebate card, visit www.myaccount.chase.com or call 866-360-3231 for more information. You will be asked to provide the authorization code set up during the activation process.
